Licensed pest control companies must report all termite contracts, wood destroying insect reports, and termite perimeter applications completed each month.
Reports are due on or before the 10th of the month following the contract or application.
You may submit your monthly reporting forms two ways:
By mail by downloading the forms below and completing with payment.
Types of reports and fees
Wood Destroying Insect Eradication Report
$12 per contract for fumigation, pretreat, post construction, spot treatment and bait treatment
$12 per contract for a combination spot and bait
$12 per inspection
Termite Perimeter Application (no fees)
Licensed pest control companies must report all pre-treatment of slabs or pier type construction.
Reports are due a minimum of 1 hour prior to beginning the application of termiticides.
There are two ways to report:
Online on our pre-treatment notification form OR
By phone to the district where the pre-treatment will occur
Information you'll need to provide (and keep a log of):
Treatment company name
Treatment structure street address and/or directions to the property
Date and time application of termiticides to the property begins
Estimated square or linear footage of each structure to be treated
Number of reported structures

Licensed pest control companies must report all structural fumigation treatments at least 24 hours prior to the beginning of treatment.
